Transfers all Medicaid eligibility determination, verification, and enrollment management functions from the Department of Human Services to the Department of Community Health effective July 1, 2027

Requires the Department of Community Health to use data analytics, audits, screenings, investigations, and prosecutions to manage Medicaid enrollment and prevent fraud, waste, and abuse

Mandates that Medicaid applicants consent to disclosure of personal information (identity, age, residence, citizenship, employment, income, wages, assets) held by state and federal agencies as a condition of applying

Authorizes disenrollment and removal of Medicaid recipients determined to be ineligible, with referral for prosecution when fraud, waste, or abuse is indicated

Requires the Department of Community Health to submit a Medicaid state plan amendment or waiver request to the federal Centers for Medicare and Medicaid Services if necessary to implement the changes
