Requires publicly owned treatment works to request PFAS-related information from industrial users by December 31, 2026, including whether they manufacture, produce, or use materials containing PFAS

Mandates industrial users provide annual disclosures starting April 1, 2027 to treatment works and the state division regarding PFAS in their operations and any best management practices implemented to reduce discharges

Applies to industrial users discharging 25,000+ gallons per day of process wastewater or contributing 5% or more of a treatment facility's capacity

Requires pretreatment permit applicants to include PFAS information in applications beginning April 1, 2027

Authorizes the Board of Natural Resources to adopt rules for implementation and requires corporate officers to certify submitted information under penalty of law
