Transfers regulation of Hawaii's medical marijuana program from the Department of Public Safety to the Department of Health, which will manage patient registration, maintain confidential medical records, and oversee cultivation and distribution.

Requires the Department of Public Safety to provide security for marijuana growing facilities, oversee transportation between counties, and distribute marijuana to qualifying patients and dispensaries.

Limits each qualifying patient to one primary caregiver at any given time and reduces the adequate supply allowance to 4 ounces of usable marijuana every 30 calendar days (down from three mature plants, four immature plants, and one ounce per mature plant).

Establishes a state-operated distribution system through medical marijuana collectives involving qualifying patients, caregivers, growers, and dispensaries statewide.

Increases the registration fee for qualifying patients from $25 to $50 and requires the Department of Health to verify patient registrations for law enforcement inquiries.
