• Defines registry access for health care providers treating patients, school personnel ensuring student compliance with immunization requirements, and authorized Department of Health personnel for public health purposes including outbreak prevention and emergency response.

• Establishes confidentiality protections requiring written consent from individuals (or parents/guardians for minors) before creating registry records, with exceptions for health care treatment reimbursement claims and court-ordered disclosure.

• Specifies registry records shall contain patient demographic information, vaccination details (types, manufacturers, lot numbers, dates administered, adverse reactions), and provider contact information, retained for 25 years after last entry.

• Provides civil liability immunity to authorized users acting in good faith and establishes misdemeanor penalties for intentional unauthorized disclosure of registry information; effective July 1, 2020.
