Transfers ownership and management of Kalawao county from the Department of Health (DOH) and Department of Land and Natural Resources (DLNR) to the Department of Hawaiian Home Lands (DHHL) when no persons with Hansen's disease remain at Kalaupapa Settlement.

Authorizes DHHL to receive transferred lands in Kalawao county, with a provision that if a sovereign native Hawaiian entity is federally and state recognized, all transferred lands and authority must be transferred to that entity.

Transferred lands shall not attain the status of Hawaiian home lands as defined in the Hawaiian Homes Commission Act, 1920.

Repeals provisions establishing the sheriff position and governance structure for Kalawao county under DOH control, transferring all administrative powers to an executive administrator elected by Hawaiian Homes Commission members.

Prohibits Maui county from exercising any authority, including zoning authority, over Kalawao county; takes effect upon approval for most provisions, with governance transition effective upon actual transfer of lands.
