Amends Hawaii Revised Statutes Section 87A-5 to modify the composition of the board of trustees of the employer-union health benefits trust fund from ten trustees appointed entirely by the governor to a split appointment structure.

Requires that of the five trustees representing public employers, four shall be appointed by the governor and one shall be appointed by the Hawaii Council of Mayors to represent the City and County of Honolulu and the counties of Hawaii, Maui, and Kauai.

Maintains the existing structure for the five trustees representing employee-beneficiaries, including three from the largest exclusive representative organizations, one from remaining organizations, and one retiree representative.

Provides counties with board representation to ensure their fiscal interests and workplace concerns are considered in benefit decisions, as they contribute substantially to the fund despite previously having no voice in board decisions.

Takes effect upon approval.
