Requires applicants to submit a substance abuse assessment from a certified counselor within 90 days before application, demonstrating they are not dependent on or abusing alcohol or drugs under applicable diagnostic standards.

Denies applications if the applicant has been arrested, convicted, or adjudicated for any traffic infraction or similar infraction in another jurisdiction since the lifetime revocation.

Limits eligibility to those with only one lifetime revocation; individuals with multiple lifetime revocations are ineligible to apply for reinstatement.

Imposes a $50 application processing fee and makes the director's decision final, though applicants may seek judicial review under the same procedures as administrative revocation appeals.
