Establishes a hospital-based screening and assessment and intensive home visitation program within the Department of Health to follow the improved healthy start program guidelines.

Program services include proactive universal screening and assessment, voluntary home visits, and referrals for families to home visitation services based on family needs.

Services shall be implemented on an incremental basis with geographic priority determined by the department's needs assessment, as funding becomes available.

Appropriates funds from the Hawaii tobacco settlement special fund for fiscal years 2011-2012 and 2012-2013 for hospital-based screening, assessment, and intensive home visitation services.

Appropriates Temporary Assistance for Needy Families federal funds for fiscal years 2011-2012 and 2012-2013 to be transferred from the Department of Human Services to the Department of Health for intensive home visitation services.
