Establishes a voluntary return-to-home program within the Department of Human Services to help homeless individuals reconnect with family and support networks in their home state.

Requires eligible participants to have a willing support system in place, proper legal clearance if on parole or probation, and demonstrate financial inability to pay for transportation.

Limits program participation to one time per individual, with participants required to sign an agreement prior to enrollment.

Authorizes the department to coordinate with airlines, cruise lines, travel agencies, homeless programs, and the visitor industry to facilitate transportation and program implementation.

Requires the department to assist participants with preparation including identification, airport transportation, security orientation, and health assessments, plus follow-up contact within one month of return to assess reunification success.
