Establishes a three-year demonstration project allowing the Department of Human Services to permit two private-pay individuals to occupy the same community care foster family home.

Requires the home to be certified for three beds, have a vacancy in the third bed for at least six consecutive months, and operators must immediately notify the department of vacancies.

Restricts private-pay pairs to individuals in specified relationships: married couples, reciprocal beneficiaries, siblings, parent-child, best friends, or civil unions recognized in Hawaii.

Requires one member of the private-pay couple to have resided in the home for at least one year and possess certified medical documentation preventing relocation.

Mandates the department monitor the project's impact on medicaid client space availability and report findings to the legislature no later than twenty days before the 2014 regular session; the act repeals three years after approval.
