Encourages the Department of Human Services to partner with an information technology provider to implement a computer application for foster youth transitioning out of foster care statewide.

Application provides secure, password-protected, web-based access to store health and education information, personal and professional contacts, and vital documents such as birth certificates, social security cards, and resumes.

Addresses the challenge of former foster youth losing vital documents during housing transitions, which creates obstacles for employment, college applications, and medical care.

Requires certified copies of the resolution to be transmitted to the Director of Human Services, Executive Director of Mental Health America of Hawai'i, and members of the Hawaii Transition Age Youth Task Force.
