Establishes the Hawaii Medicaid Modernization and Innovation Task Force with 35 members representing healthcare providers, patients, insurers, and state agencies to develop and implement a patient-centered health home pilot program for Medicaid enrollees.

Task force must adopt definitions, criteria, and standards for health homes; certify participating facilities; develop quality measures and payment methodologies including per member per month care coordination fees; and establish annual reporting requirements.

Task force shall submit a program development report to the legislature by 20 days prior to the 2012 regular session and an implementation report by 20 days prior to the 2013 regular session, then cease to exist on June 30, 2013.

Authorizes establishment of an advance health-care directive program to encourage Hawaii residents to maintain copies of their directives on their person.

Requires managed care plans and QUEST program providers to give 90 days' prior written notice before modifying reimbursement policies and prohibits retroactive reductions to previously approved reimbursements; effective July 1, 2050.
