Establishes a two-year childhood obesity pilot program under the Department of Health, working with Hawaii Primary Care Association to develop parameters and implementation guidelines.

Defines childhood obesity as a body mass index in the ninety-fifth percentile or above for a person under eighteen years of age, considering factors such as body frame and muscularity.

Requires the Department of Health to select twenty health care providers to participate, including at least one each of solo practitioners, group practitioners, pediatricians, registered dietitians, internal medicine doctors, psychiatrists, psychologists, and registered nurses.

Mandates all individual and group insurers in Hawaii provide coverage for childhood obesity treatment under the pilot program for insured individuals and their dependents, with notice required to insureds by December 31, 2012.

Requires pilot program providers to submit a joint report analyzing operational successes and challenges to the Department of Health and Legislature no later than twenty days before the 2015 regular session convenes, effective July 1, 2040.
