Establishes registration requirements for athletic trainers in Hawaii, making it illegal for individuals to practice athletic training or use the title "athletic trainer" without departmental registration effective August 1, 2011.

Requires athletic trainers to maintain current certification from the Board of Certification, Inc., complete educational requirements, and renew registration every three years with the Department of Commerce and Consumer Affairs.

Mandates that athletic trainers work under the supervision of a treating physician who provides verbal or written orders and oversight of athletic training services.

Provides exemptions for licensed healthcare providers (physicians, occupational therapists, physical therapists), students in training programs, armed forces personnel, temporary visiting professionals, and first aid providers.

Authorizes the Director of Commerce and Consumer Affairs to deny, revoke, or suspend registration for grounds including false applications, professional misconduct, impairment, ethics violations, and failure to report disciplinary actions from other jurisdictions; establishes civil penalties of up to $1,000 per violation.
