Extends the physician workforce assessment fee from June 30, 2012 to July 1, 2015, with physicians and surgeons continuing to pay $60 per license renewal.

Requires 100 percent of assessment fees deposited into the John A. Burns School of Medicine special fund be used for: addressing federal health care reform implementation, supporting the medical home model, and supporting increases in physician reimbursements.

Limits annual expenditures from the special fund to no more than $150,000, including maintaining a secure database of physician workforce assessment information with disclosure only upon express written physician consent.

Requires the John A. Burns School of Medicine to submit annual workforce assessment reports to the legislature, state health planning agency, and Hawaii medical board (changed from biannual reporting), including fee expenditure details.

Effective July 1, 2011.
