Expands infertility treatment coverage in health insurance policies to include procedures beyond in vitro fertilization, such as embryo transfer, intrauterine insemination, gamete intrafallopian transfer, zygote intrafallopian transfer, and intracytoplasmic sperm injection, along with diagnosis, diagnostic tests, medications, and surgery.

Requires coverage of at least four completed fresh cycles per lifetime, with frozen embryo transfers not counting as additional cycles against this limit.

Removes the requirement that patients be married to receive infertility treatment coverage, expanding eligibility to include unmarried individuals and same-sex couples.

Allows insurers to limit coverage for certain procedures (in vitro fertilization, gamete intrafallopian transfer, and zygote intrafallopian transfer) to women age 45 and younger.

Applies to all individual and group health insurance policies that provide pregnancy-related benefits and require procedures to be performed at facilities meeting American Society for Reproductive Medicine standards or Society for Assisted Reproductive Technologies membership.
