Requires medicaid and contracted health plans to reimburse acute care hospitals at acute care rates for patients occupying hospital beds while on waitlists for long-term care facilities.

Mandates that long-term care facilities caring for patients with medically complex conditions receive reimbursements at least equal to subacute care rates, particularly for patients transitioning from acute care.

Defines "medically complex condition" as a combination of chronic physical conditions requiring ongoing technological, pharmacological, and therapeutic interventions.

Eliminates barriers preventing medicaid funds from flexibly following patients as they transition from acute care to community-based or private institutions.

Appropriates unspecified general revenues for fiscal year 2011-2012 to cover increased medicaid reimbursement costs under this Act, with implementation effective July 1, 2050.
