Establishes a formula for calculating state contributions toward Medicaid recipients' attorney's fees and costs when obtaining judgments, settlements, or awards in lieu of recovering full amounts.

Requires the Department of Human Services to accompany liens for Medicaid reimbursement with an itemized list of payments made by the department.

Caps the department's lien at one-third of the settlement, judgment, or award after deducting the state's contribution for attorney's fees and expenses.

Authorizes the Department of Human Services to develop fair and equitable reimbursement levels for nursing facility residents who are Medicaid recipients and an equitable referral system for Medicaid clients transferring from hospitals to community-based care homes.

Requires the Department of Human Services to provide written notice of lien to the court when restitution is sought from a third person in a criminal proceeding.
