Allows employees to request wage and employment information from their employer regarding temporary disability insurance claims, in addition to insurers' existing right to request such information.

Requires employers to submit requested information to the requestor within seven days of receiving the request.

Increases penalties for failing to submit information timely to $250 for a first delinquent request and $500 for a second or subsequent delinquent request (previously $10 per delinquent request).

Penalty exemption applies when employer demonstrates good cause for late filing.

Takes effect upon approval.
