Establishes the med-QUEST patient-centered medical home initiative to improve health care delivery under state medicaid plans.

Addresses uncoordinated care that results in duplicative procedures, conflicting treatment recommendations, unnecessary hospitalizations, and adverse drug reactions.

Requires the Department of Human Services to present an overview of the program's design to the Legislature.

Appropriates funds for coordination and evaluation of the patient-centered medical home model.

Sets effective date of July 1, 2050 to allow for further legislative discussion on the measure.
