Requires all clean and sober homes and halfway houses to obtain licenses from the Department of Health beginning July 1, 2012, to protect residents, staff, and neighbors.

Mandates operators obtain a general excise tax license and conduct at least one public informational meeting at a neighborhood board meeting before receiving a license, with written notification sent to adjoining property owners.

Directs the Department of Health to adopt rules establishing standard operating procedures, occupancy standards based on room size, community communication best practices, and monitoring compliance.

Establishes an annual licensing fee with proceeds funding administrative costs and requires the Department of Health to maintain a public listing of all licensed facilities.

Requires state agencies, including the Hawaii Paroling Authority, to refer clients only to licensed facilities after June 30, 2013, and gives existing facilities one year from June 1, 2012, to comply with licensing requirements.
