Requests the Department of Human Services and Department of Health to develop a two-year pilot demonstration program targeting the top 5% of Medicaid beneficiaries whose care accounts for a significant portion of state Medicaid expenditures.

Requires the pilot program to coordinate services for high-need, high-cost Medicaid beneficiaries and include comprehensive outpatient health care and social services components.

Directs the pilot to identify methods for reducing potentially avoidable costs from care defects and emergency room admissions.

Requires the departments to report to the Legislature on the pilot program's status no later than 20 days before the 2014 Regular Session convenes.
