Requires children to present a report from a licensed dentist showing completion of a dental examination within one year before first entering elementary, middle, intermediate, or high school.

Allows provisional school entry with written documentation that the child is undergoing a dental examination, with completion required within three months of enrollment.

Extends the notification period for non-compliance from 30 days to 90 days before a child is excluded from school for incomplete physical or dental examinations.

Requires the Department of Education to provide student health record forms for dental examinations to schools, private dentists, and health department personnel.

Effective July 1, 2050, applying beginning with the 2015-2016 school year.
