Requires the Department of Health, in collaboration with the Department of Human Services, to identify goals and benchmarks and develop departmental plans to reduce diabetes incidence, improve diabetes care, and reduce complications.

Mandates an annual report to the legislature no later than 20 days before each regular session detailing the health, societal, and financial impacts of diabetes on the State, counties, and both departments.

Annual report must include the number of persons with diabetes, participation rates in diabetes education and treatment programs, program costs including Medicaid, and comparative costs of diabetes treatment versus other chronic diseases.

Requires reporting on the effectiveness of diabetes education and treatment activities, funding sources and amounts appropriated by the legislature, and the level of coordination between the two departments.

Annual report must include any proposed legislation to reduce diabetes and pre-diabetes impacts in Hawaii, with recommended appropriations and expected outcomes identified.
