Requires the Department of Health to maintain an inventory of skilled nursing facilities, intermediate care facilities, assisted living facilities, developmental disabilities domiciliary homes, special treatment facilities, therapeutic living programs, adult residential care homes, and adult independent group residences licensed or certified under its jurisdiction.

Mandates the Department of Health maintain a current list of all vacancies in these facilities to facilitate placement of individuals and requires all listed facilities to report vacancy information on a regular basis.

Requires the Department of Health to maintain an inventory of community care foster family homes certified under its jurisdiction and a current list of their vacancies, with regular vacancy reporting by all certified homes.

Requires the Department of Human Services to maintain an inventory of community care foster family homes certified under its jurisdiction and a current list of their vacancies, with regular vacancy reporting by all certified homes (effective July 1, 2014, and repealed July 1, 2014).

Addresses the lack of centralized information about facility vacancies that currently makes placement of individuals with disabilities in community-based settings labor-intensive, consistent with the nationwide deinstitutionalization trend following the Olmstead v. LC Supreme Court decision.
