Establishes the Makua Valley Reserve Commission consisting of seven members, including representatives from Malama Makua, Hui Malama O Makua, evicted families, the Waianae coast community, the Office of Hawaiian Affairs, the Board of Land and Natural Resources, and Native Hawaiian organizations.

Reserves Makua Valley (three ahupuaa: Kahanahaiki, Makua, and Koiahi) in perpetuity for Native Hawaiian cultural, spiritual, and subsistence practices; archaeological, historical, and environmental preservation; habitat restoration; and education, with commercial uses strictly prohibited.

Grants the commission oversight authority over the Department of Land and Natural Resources regarding control and management of the valley reserve, including establishing criteria and policies, approving contracts and rules, and providing advice on federal conveyance and coastal zone matters.

Creates the Makua Valley Rehabilitation Trust Fund to receive federal funds and legislative appropriations for environmental restoration and rehabilitation projects, with the fund repealing on July 1, 2045.

Provides for transfer of management and control to a sovereign Native Hawaiian entity upon its recognition by the United States and the State; Act takes effect July 1, 2050.
