Adds a new objective to Hawaii's State Planning Act requiring elimination of health disparities by identifying and addressing social determinants of health.

Establishes a state policy to prioritize programs, services, and interventions addressing social determinants of health to improve Native Hawaiian health and well-being consistent with federal law (42 U.S.C. § 11702).

Requires prioritization of health programs targeting disproportionately affected demographic groups including Native Hawaiians, other Pacific Islanders, and Filipinos, with review and revision of affected groups allowed every ten years based on epidemiological data.

Recognizes that social determinants of health are the most significant factor in health outcomes according to the World Health Organization, Centers for Disease Control and Prevention, and federal Healthy People 2020 Initiative.

Takes effect upon approval.
