Requires children to undergo a physical examination by a licensed physician or advanced practice registered nurse within twelve months before or after attending seventh grade, beginning with the 2015-2016 school year.

Modifies existing requirements for first-time school attendance to use the term "attend" instead of "be admitted to" and extends the deadline for submitting completed physical examination documentation from initial submission to no later than three months after first attendance.

Allows children to provisionally attend school while undergoing a physical examination upon submission of documentation from a licensed physician, advanced practice registered nurse, or authorized representative of the department of health.

Appropriates an unspecified amount from general revenues to the department of education for fiscal year 2014-2015 to implement the requirements of this Act.

Takes effect on July 1, 2030.
