Establishes a Comprehensive Maternal and Child Health Quality Improvement Program within the Department of Health to improve statewide coordination of infant mortality reduction planning and oversee implementation of evidence-based health practices.

Creates a Hawaii Maternal and Child Health Quality Improvement Collaborative consisting of 13 voting members appointed by the governor plus two ex-officio voting members (directors of health and human services) to approve the strategic plan and provide recommendations on reducing infant mortality.

Requires the Department of Health to develop and publish a statewide Comprehensive Maternal and Child Health Quality Improvement Strategic Plan by January 1, 2016, with strategies addressing social determinants of health related to reducing infant mortality.

Mandates each hospital establish written policies against non-medically indicated inductions or cesarean sections prior to 39 weeks of gestation consistent with American College of Obstetricians and Gynecologists guidelines.

Requires all licensed birthing facilities to report perinatal core measure set data to the Department of Health as required by Centers for Medicare and Medicaid Services or Joint Commission standards.
