Changes chapter 327K terminology from "Physician Orders for Life-Sustaining Treatment" (POLST) to "Provider Orders for Life-Sustaining Treatment" throughout the statute.

Expands authority to execute POLST forms to include advanced practice registered nurses in addition to licensed physicians, allowing these healthcare providers to sign POLST forms directly with patients.

Replaces the term "surrogate" with "legally authorized representative" throughout chapter 327K to align with terminology used in chapter 327E, the Uniform Health-Care Decisions Act.

Adds a new definition of "legally authorized representative" to include agents, guardians, surrogates, and those designated through a power of attorney for health care as defined in chapter 327E.

Takes effect July 1, 2014.
