Prohibits tanning facility owners, lessees, and operators from allowing anyone under age 18 to use tanning equipment, effective July 1, 2014.

Requires tanning facilities to post a conspicuous notice addressing the legal ban, associated health risks, penalties, and procedures for reporting violations.

Establishes penalties of up to $250 for a first violation and up to $500 for each subsequent violation, enforced by the director of health.

Exempts licensed physicians using ultraviolet equipment for medical diagnostic and therapeutic purposes, and individuals with tanning equipment for personal, noncommercial use.

Authorizes the director of health to adopt rules implementing the requirements under chapter 91 of Hawaii Revised Statutes.
