Department of Education is requested to administer a Youth Risk Behavior Survey to students in grades 6-12 attending public schools, including charter schools

Private schools are requested to administer the survey to their students in grades 6-12

Survey is to be administered only in odd-numbered years

Students may be excused from participation if their parent or legal guardian submits written notification withholding permission

Survey monitors health-risk behaviors including weight and diet, physical activity, injury and violence, tobacco use, alcohol and drug use, and sexual behaviors
