Request the Department of Transportation to develop and issue special identification devices such as medical identification cards, jewelry, or other alternatives that allow cardholders to provide information about their specific medical conditions for emergency situations.

Consider multiple forms of special identification including jewelry, license plates, name plates, pins, wheelchair stickers, and other necessary forms.

Consider implementing a provision on Hawaii state identification cards indicating that the holder also has a special identification card or device.

Address the needs of approximately 32,000 Hawaii adults with serious mental illness, 12,000 children with serious mental conditions, and elderly people with Alzheimer's disease or dementia who may have difficulty conveying medical information in emergencies.

Transmit certified copies of the resolution to the Director of Health, Director of Transportation, and Director of the Alzheimer's Association Aloha Chapter.
