Requires tanning facilities to provide written notice to customers before initial use and annually thereafter, detailing health risks including eye damage, skin burns, premature aging, skin cancer, and photosensitizing reactions to medications and cosmetics.

Mandates prominently displayed warning signs in tanning areas addressing exposure risks, eye protection requirements, and potential chronic skin damage from repeated use.

Establishes operational requirements including trained operators on-site during business hours, sanitized equipment after each use, mandatory protective eyewear, accurate timing devices controlled by operators, temperature limits (100°F maximum), and restricts use to once per 24 hours with manufacturer-recommended exposure times.

Authorizes the director of health to impose fines for violations and adopt implementing rules; exempts licensed physicians using medical diagnostic/therapeutic equipment and personal, noncommercial tanning equipment; effective July 1, 2050.
