Requires all children to present written documentation of a physical examination completed within 12 months before attending seventh grade, beginning with the 2015-2016 school year, performed by a licensed physician or advanced practice registered nurse.

Requires children attending school in Hawaii for the first time to present documentation of a physical examination performed within one year of attendance, with provisional attendance allowed if examination is in progress and completed within three months.

Directs the Board of Education to establish a pilot program in one complex area requiring physical examinations within six months before seventh grade attendance, beginning in 2015-2016, and to report findings and recommendations to the legislature by 2016.

Requires the Department of Health to establish or partner to administer a school-based dental sealant program at a high-need demonstration school and submit a report to the legislature by 2015 detailing progress toward statewide implementation and the Healthy People 2020 goal of 28.1% of six to nine-year-old children having molar sealants.

Appropriates funds to the Department of Education for fiscal year 2014-2015 to implement the physical examination requirements and pilot program, and to the Department of Health for the dental sealant demonstration program.
