Require primary care physicians and pediatricians to administer annual body mass index measurements to all patients ages two to eighteen years old.

Mandate that physicians report anonymous age and body mass index statistics annually to the Hawaii health information exchange to maintain a statistical database for public health purposes.

Require all individual and group health insurance policies (accident and health insurance, hospital and medical service corporation contracts, and health maintenance organization plans) to cover the cost of annual body mass index measurements for insured persons ages two to eighteen years old.

Define body mass index as 703 times a person's weight in pounds divided by the square of the person's height in inches.

Apply mandatory insurance coverage requirements to all health plan contracts issued or renewed on or after January 1, 2015, with the Act's general effective date set for July 1, 2050.
