Establishes a Medicaid Buy-in Program within the Department of Human Services for workers with disabilities, effective July 1, 2017, with a minimum enrollment of 200 individuals.

Requires participants to be Hawaii residents at least 18 years old with a Social Security Administration-recognized disability, gainfully employed, have income not exceeding 300% of federal poverty level, and assets not exceeding a department-set amount of at least $25,000.

Creates a temporary Medicaid Buy-in Pilot Program within the Department of Health serving 100 workers with disabilities who lost Medicaid eligibility due to employment, operating from July 1, 2015, to June 30, 2017.

Authorizes the Department of Human Services to require program participants to pay premiums or cost-sharing charges as determined by the department.

Appropriates funds to the Department of Health for the pilot program and to the University of Hawaii Center on Disability Studies for outreach and training related to the Medicaid Buy-in Program.
