Extends the Governor's Health Care Transformation Program from June 30, 2015 to June 30, 2017, then transfers all functions to the Hawaii Health Care Planning and Policy Authority within the Department of Health effective July 1, 2017.

Creates the Office of Health Care Transformation within the Hawaii Health Care Planning and Policy Authority, headed by a Health Care Transformation Officer appointed by the Governor, with authority to oversee statewide health care transformation activities including quality improvement, payment models, health information technology coordination, and workforce development.

Establishes the Health Care Transformation Special Fund to receive grants, fees, donations, and other revenues for health care transformation activities, and renames the State Health Planning and Development Agency to the Hawaii Health Care Planning and Policy Authority.

Creates the State Innovation Waiver Task Force to develop a health care reform plan and draft application for a federal state innovation waiver as an alternative to the health insurance exchange, with a final report due to the legislature no later than twenty days before the 2016 regular session; task force dissolves June 30, 2017.

Appropriates funds for the healthcare transformation coordinator for fiscal year 2014-2015 and for the state innovation waiver task force operations, with Part II provisions taking effect July 1, 2050.
