Creates a medical release pilot program allowing the Department of Public Safety to refer inmates to the Hawaii Paroling Authority for early release based on medical grounds.

Establishes three eligibility criteria: inmates with terminal illness (death probable within one year), seriously debilitating irreversible conditions manageable in community settings, or serious age-related conditions requiring costly or complex care.

Requires medical release requests to include a physician's report on diagnosis and prognosis, director's assessment of violence/recidivism risk, and a comprehensive medical release plan ensuring continuity of care.

Prohibits medical release for inmates posing danger to society and requires paroling authority supervision and reasonable conditions through sentence expiration.

Sunsets the pilot program on June 30, 2016 with an effective date of July 1, 2050.
