Request the Department of Education to administer the Youth Risk Behavior Survey to students in grades six to twelve in select public schools

Request the State Public Charter School Commission to administer the survey to students in grades six to twelve in select charter schools, with private schools given the opportunity to participate

Survey to be administered only in odd-numbered years and monitor health-risk behaviors including weight and diet, physical activity, injury and violence, tobacco use, alcohol and drug use, and sexual behaviors

Allow parents or legal guardians to opt their child out of the survey by submitting written notification to the school

Data from the survey is intended to measure progress toward health objectives, assess trends in health-risk behaviors, evaluate intervention impacts, and track progress toward school health program goals
