Department of Education is requested to administer a youth risk behavior survey to students in grades 6-12 in public schools, including charter schools.

Private schools are requested to administer the same survey to their grades 6-12 students.

Survey administration is limited to odd-numbered years only.

Students may be excused from participating if their parent or legal guardian submits written notification withholding permission.

Survey monitors health-risk behaviors including weight and diet, physical activity, injury and violence, tobacco use, alcohol and drug use, and sexual behaviors.
