Establishes a three-year pilot program (July 1, 2015 - June 30, 2018) administered by the Department of Human Services to educate and assist persons in applying for supplemental nutrition assistance program benefits.

Requires the department to allow online submission of supplemental nutrition assistance program applications including supporting documents.

Provides performance-based incentive payments to organizations that assist applicants with pre-screening, completing application forms, gathering documentation, and delivering applications to the department.

Includes additional incentives for organizations whose assisted applicants properly submit applications online and are subsequently approved for benefits.

Requires annual reports to the legislature (2016, 2017, 2018) detailing participating organizations, applications processed, incentive payments made, enrollment data, and program evaluations.
