Appropriates $4,800,000 for fiscal years 2015-2016 and 2016-2017 to restore basic adult dental benefits for medicaid enrollees, with maximum federal matching funds sought.

Appropriates $800,000 for each of fiscal years 2015-2016 and 2016-2017 to provide outreach and eligibility services for medicaid and children's health insurance program enrollment at federally qualified health centers.

Appropriates $1,000,000 from the community health centers special fund for each fiscal year to establish health homes for medicaid enrollees at federally qualified community health centers under the Affordable Care Act.

Appropriates $1,000,000 from general revenues for each fiscal year to provide administrative support for the health homes program.

Appropriates $6,000,000 for each of fiscal years 2015-2016 and 2016-2017 for direct primary medical, dental, and behavioral health care to uninsured and underinsured patients at community health centers, distributed on a per-visit or per-member per-month basis.
