Requires health insurance policies issued or renewed after December 31, 2015, to cover medically necessary orthodontic treatment for orofacial anomalies (cleft lip, cleft palate, and related birth defects) for individuals under 26 years old.

Sets maximum benefit of $5,500 per treatment phase with no limits on number of orthodontist visits; benefit adjusted annually for inflation using Consumer Price Index after December 31, 2016.

Applies to commercial accident and health insurance, hospital/medical service plans, and health maintenance organizations, but excludes limited benefit health insurance.

Requires insurers to provide written notice to policyholders about coverage by December 31, 2016, and permits standard copayments, deductibles, and coinsurance applicable to other medical services.

Known as "Anya's Law"; takes effect July 1, 2015, based on 2014 state auditor findings that coverage provides substantial social benefit with minimal insurer cost.
