Requires seventh grade students to present written documentation of a physical examination performed within twelve months before attendance, effective with the 2016-2017 school year.

Allows provisional school attendance if a student submits documentation that a physical examination is in process, with completion documentation due within three months.

Maintains existing requirement that students attending school for the first time in Hawaii must provide physical examination results from a licensed physician or advanced practice registered nurse within one year of enrollment.

Appropriates funds for fiscal years 2015-2016 and 2016-2017 to the Department of Education for recordkeeping, record review, accountability, and parent notifications related to mandatory seventh grade physical examinations.

Takes effect July 1, 2050.
