Extends the Nursing Facility Sustainability Program for one additional year, with program expiration date changed from December 31, 2015 to December 31, 2016.

Maintains the nursing facility sustainability fee structure at $13.46 per resident day for most facilities and $5.85 per resident day for qualifying facilities described in subsection (d)(2).

Requires at least 88 percent of fee revenue to match federal Medicaid funds for enhanced capitated rates to Medicaid managed care health plans, with 12 percent available for other departmental purposes.

Provides $14,000,000 appropriation from the nursing facility sustainability program special fund for fiscal year 2015-2016 to support increases in Medicaid payments to private nursing facilities.

Changes baseline funding reference date from June 30, 2014 to June 30, 2015 for purposes of determining when fee collection should be discontinued if state funding for nursing facility services is reduced.
