Prohibits the use of tobacco products and electronic smoking devices by any person on premises of all Hawaii Health Systems Corporation (HHSC) health facilities statewide, subject to federal law or regulation.

Defines "premises" to include all indoor and outdoor areas within state-designated property boundaries of HHSC facilities, including employee and visitor parking lots.

Requires HHSC to educate all health facility personnel, contractors, patients, and visitors about the prohibitions and display prominent signage stating "tobacco and electronic smoking device use is prohibited" at entrances and throughout outdoor premises in compliance with section 328J-8.

Mandates that any prohibition affecting public employees must be subject to consultation pursuant to section 89-9(c) with exclusive representatives of public sector collective bargaining units.

Takes effect on July 1, 2050.
