Requests convening of a Family Caregivers Working Group to examine issues related to family caregivers of hospital-discharged patients, including caregiver designation, notification, discharge planning involvement, and hospital instruction roles.

Working group membership includes 15 participants comprising state officials, hospital representatives, caregiver advocacy organizations (AARP Hawaii, Kokua Council, Hawaii Family Caregiver Coalition), and legislative co-chairs appointed by the Senate President and House Speaker.

Directs co-chairs to listen to stakeholder discussions, report on areas of agreement and concerns, analyze community resources, and make recommendations without voting on findings.

Dissolves previous working group created by H.C.R. No. 78 from 2014 that failed to reach consensus and submit required report.

Sets December 1, 2015 as the dissolution date for the new working group and authorizes Legislative Reference Bureau to provide logistical support and draft reports.
