Director of Health is requested to convene a task force to assess the scope and feasibility of establishing a statewide system or protocol for accessing advance care planning forms across all health facilities in Hawaii.

Task force shall focus on documents including health care power of attorney, provider's orders for life-sustaining treatment, advance health care directives, and other forms the Director deems relevant.

Director of Health or designee shall serve as chairperson and select task force members to include representatives from hospitals, health care facilities, nursing homes, hospital administration attorneys, and the University of Hawaii Elder Law Program.

Task force must submit findings, recommendations, and any proposed legislation to the Legislature no later than twenty days before the 2016 Regular Session convenes.

Task force shall be dissolved on June 30, 2017.
