Requires the department of agriculture to establish a mandatory pesticide disclosure program for all persons or entities that cultivate crops beginning July 1, 2016.

Mandates monthly reporting of pesticide use including trade name, registration numbers, active and inert ingredients, application method, date, quantity, geographic location by address and tax map key number, and acreage treated.

Requires the department to post online a monthly summary of all pesticide use reports and documentation of non-compliance beginning October 1, 2016.

Increases the pesticide use revolving fund balance ceiling from $250,000 to $500,000.

Effective date is July 1, 2050.
